Prior to your first visit to the offices of Dr. Gura, we ask that you download and fill out the forms below. Doing so will save you time when arriving at our office for your consultation. If you do not fill out the forms beforehand, we ask that you arrive 15 minutes before your scheduled appointment in order to have the time necessary to fill them out.
