Appointments
Dr. Gura sees patients by appointment only. If you would like to schedule an appointment please call (310) 734-5551 between the hours of 9am and 5pm Monday through Friday. For your initial visit, we ask that you download and fill out the forms in our Patient Forms section and bring them with you to your appointment. If you are unable to do so, please arrive at least 15 minutes early for your appointment so that you can fill out the forms here in the office.
What to Bring to Your First Visit
In addition to the New Patient Forms, please bring the following with you to your appointment:
- A complete list of all medications that you are taking along with dosages and how often you take them. This includes both prescription and OTC (over the counter) medications as well as any dietary supplements.
- Insurance Card
- Photo ID
- Co-pay (if applicable)
- Referral from your primary care physician if necessary
Appointment Cancellations
Our office will call to confirm your appointment one business day prior to your visit. If for some reason you need to cancel or reschedule your appointment, please call our office at least 24 hours in advance to let us know.
